You have the right to read your medical notes. If you wish to read your medical notes please enquire at reception.
Chaperones
In general practice we do not have the personnel resources to provide chaperones during every physical examination. However, if you would prefer to have a member of staff to chaperone you during your visit then please inform the receptionist on arrival.
Confidentiality
The NHS takes great care to ensure your information remains confidential and our staff are trained to maintain confidentiality. You can ask to speak to a receptionist in a private room if you wish.
Your personal information and consultations are private and will not be revealed to anyone outside the NHS without your permission.
Within the NHS, we only share the minimum amount of information necessary and only with people who have a genuine need to know.
When we use information about you, wherever possible, we remove any details that could identify you.

Medical Reports
The NHS provides for most services but for some services a fee is charged. These range from pre-employment medicals, private insurance claims for sickness benefits, fitness to drive, certifying passport photographs, holiday cancellation etc. A list of fees is displayed at the reception desk. Please note special appointments are necessary for all medicals.
Teaching
There will be at times doctors or nurses in training at the surgery. You may be asked whether you will see them but are under no obligation to do so.
