You can register with the practice providing you live within the practice boundary (S3, S4, and S5). If you have a NHS Medical Card please take this to reception. If not, the receptionist will give you a form to complete.

It is often necessary for us to confirm that you are eligible for free NHS care and you may be asked to provide proof of your identity and eligibility.

Your form will then be sent to the NHS Sheffield who would arrange for your medical notes to be transferred from your previous doctor; this would normally take 7– 8 weeks.

You will be required to attend for a New Patient Check with the Nurse or Health Care Assistant.

If you have a family member staying with you who needs medical attention, they can be seen by a doctor as a temporary patient.
